Health information and HIPAA notice

This notice explains how Allied Medical Management LLC ("Allied", "we" or "us") handles health information in connection with alliedmedical.supplies (the "site"), our forms and our email. It also serves as our consumer health data privacy policy for the purposes of Washington's My Health My Data Act, Nevada's consumer health data law and similar state laws.
Summary
- Allied is a wholesale distributor. We sell to businesses, not to patients, and we do not need health information about anyone to quote, sell or ship products.
- Do not send us patient information or any other health information, about yourself or anyone else, through our forms, by email, by phone or by mail.
- Allied is not a HIPAA covered entity or business associate for anyone who uses the site or sends us information through it.
- We do not intend to collect consumer health data, we do not sell it, and we do not use it. If it reaches us, we delete it.
What not to send
Do not include any of the following in a quote request, contact message, email or other message to us:
- a patient's name, date of birth, address, phone number or photo;
- a diagnosis, condition, symptom, test result or prescription;
- insurance, Medicare or Medicaid numbers, or medical record numbers;
- body measurements or fitting details tied to a named person; and
- any other information about the health of you or another person.
A quote request needs only products, sizes, quantities and your business details. If you need to report a problem with a product, describe the product, its lot or serial number and what happened, without identifying a patient. This site and our forms are not for medical emergencies. If someone needs urgent medical help, call 911.
HIPAA
HIPAA applies to health plans, health care clearinghouses and health care providers that conduct certain electronic transactions, and to the business associates that handle protected health information for them. Allied sells equipment and supplies to businesses at wholesale. We do not bill Medicare, Medicaid or any insurer, and we do not dispense products to patients.
- Allied is not a HIPAA covered entity.
- Allied does not act as a business associate for anyone who uses the site or sends us information through it, and the site is not designed to receive or protect protected health information (PHI).
- We do not sign business associate agreements for use of the site, our forms or our email.
- If your business is a covered entity or business associate, do not disclose PHI to us.
If health information reaches us
If we find that a form submission, email or other message contains patient information or other health information, we will not use it for any purpose. We will delete it from our website database and our email as soon as we find it, and we may ask you to resend your request without it. Copies in our backups are removed as those backups expire, generally within 30 days. [CONFIRM]
Consumer health data
Washington, Nevada and some other states regulate "consumer health data", which is personal information that is linked or reasonably linkable to a person and identifies that person's past, present or future physical or mental health. This section describes how we handle it.

Your rights
Wherever you live, you may ask us to:
- confirm whether we have collected, shared or sold health information about you;
- give you a copy of it, and a list of any third parties or affiliates it was shared with and how to contact them;
- delete it, including from backups; and
- stop any further collection or sharing, or withdraw any consent you gave.
We do not charge a fee, and we will not treat you differently for making a request.
How to make a request

- Write to Allied Medical Management LLC, Attention: Privacy, 38 Corbin Pl, Brooklyn, NY 11235, USA.
We will confirm your identity before we act on a request, as described in our Privacy policy. We will respond within 45 days after we receive your request. If we need more time, we may extend this once by up to 45 more days and will tell you why within the first 45 days. We will also pass a deletion request on to any service provider that holds the information.
If we deny your request, you may appeal by emailing us with "Health data appeal" in the subject line. We will send a written decision, with our reasons, within 45 days after we receive your appeal. If we deny the appeal, we will tell you how to contact your state attorney general to submit a complaint.
Changes to this notice
When we update this notice, we will post the new version on this page and change the dates at the top. If we ever decide to collect or use consumer health data, we will update this notice first and ask for any consent the law requires.
